  Midi Health recommends contacting your insurance provider to understand your plan’s benefits and responsibilities, including your in and out-of-network coverages.

   

  You can also ask your insurance carrier for a Summary Plan Description (SPD) at any point in time if you need detailed coverage information.

• How do I sign up for a visit?
