Q. What kind of care does Herself Health provide?
A: Herself Health operates five clinics in the Minneapolis-St. Paul area, providing primarily in-person primary care, with virtual visits also available.
Herself Health is in-network with most Blue Cross Blue Shield PPO plans. As their partner, Midi visits are also covered for most Minnesota residents with these health plans. Midi visits are always virtual and conducted by Midi-trained clinicians.
PARTNERSHIP
Q: What is the partnership between Midi Health and Herself Health?
A: Midi Health has partnered with Herself Health to expand access to insurance-covered virtual care in Minnesota. Thanks to this partnership, Midi visits are covered for most Minnesota residents with primary insurance through in-network Blue Cross Blue Shield PPO health plans.
If you have one of these plans on file with Midi and live in the state of Minnesota:
- Herself Health will handle billing insurance for your Midi visit on our behalf. (Coverage varies by plan; deductibles, coinsurance, and copays may still apply.)
- When you receive your Explanation of Benefits (EOB) related to Midi visits, HERSELF HEALTH will be listed as the service provider.
- Bills for any cost-sharing you are responsible for under the terms of your health plan will come from Herself Health, not Midi.

SCHEDULING
Q: Does Midi’s relationship with Herself Health impact what clinicians I can see?
A: In order for your Midi visit to be covered by a primary in-network Blue Cross Blue Shield PPO plan, your Midi clinician must be considered an in-network provider under that plan.
- Visits scheduled before 2/19/26 for 2/19/26 or later: Our team will check to make sure the clinician you’re scheduled with is in network with your BCBS plan. If they are not, you will receive a text and email offering you the choice to either reschedule with an in-network clinician or switch to our self-pay billing option.
Going forward, our portal will only give you the option to book with clinicians who are in-network with your plan. If you want to continue seeing a particular clinician who is not in-network with your plan, you can switch to self-pay.
CARE COORDINATION
Q: I have decided to visit a Herself Health clinic for in-person care. Will my Midi clinician communicate with them to coordinate my care?
A: Patients who opt to get virtual care from Midi and also seek care from Herself Health will benefit from integrated care.
Through secure health information sharing via your electronic medical record (EMR), Herself Health providers will be able to view Midi-collected medical information, visit notes, and Care Plan details. Likewise, Midi clinicians will be able to view Herself Health-collected medical information, visit notes, and test results.
Q: Will the medical information Midi has on file for me be shared with Herself Health if I go to one of their clinics for in-person care?
A: Yes. Midi will securely share medical information, Care Plan details, and visit notes with Herself Health so they are aware of the care you are already receiving. Herself Health will also share medical information they collect as well as information on the care they provide (e.g., visit notes, test results) with Midi.
HERSELF HEALTH SERVICES
Q: I’m interested in receiving primary care services from Herself Health. How do I learn more or enroll?

Is there a fee to access their services?
A: Herself Health offers a specially designed primary care experience for women 50+ that goes beyond a traditional primary care clinic. Herself Health has longer appointments, a more personal approach, and a strong focus on prevention and whole-person care.
To support these enhanced services, patients with commercial insurance (non-Medicare) pay a fee to enroll in Herself Health’s Membership program for primary care. This fee is waived for patients on Medicare or other government-sponsored health insurance plans.

Q: Why did I get a bill from Herself Health?
A: Most Minnesota residents with Blue Cross Blue Shield PPO primary insurance can have their Midi visits covered as in-network care under their health plan thanks to our partnership with Herself Health.
Herself Health handles billing these patients on behalf of Midi. (Coverage varies by plan; deductibles, coinsurance, and copays may still apply.)
If you live in MN and have a primary in-network Blue Cross Blue Shield PPO plan on file with us:
- Your Explanation of Benefits (EOB) related to Midi visits will list HERSELF HEALTH as the service provider.
- Bills for any cost-sharing you are responsible for under the terms of your health plan will come from Herself Health, not Midi.

Q: I live in MN and have in-network Blue Cross Blue Shield PPO insurance. Why isn’t the bill for my Midi visits coming from Herself Health?
While it is true that Herself Health handles billing for most MN patients with primary in-network Blue Cross Blue Shield PPO insurance on our behalf, MN patients who get this insurance through one of Midi’s employer partners will continue to be billed by Midi.

Q: Will I get two bills? One from Herself Health, one from Midi?
A: You will only get one bill for your Midi visit. Who you get the bill from depends on your insurance:
If you reside in Minnesota and have a primary, in-network Blue Cross Blue Shield PPO plan on file with Midi, you will only receive a bill for any cost-sharing from Herself Health. (See the above question to learn about one exception.)
If you reside in Minnesota and have different primary, in-network insurance on file with Midi, you will receive a bill for any cost-sharing from Midi.
If you have in-network insurance and do not live in Minnesota, your bill will come from Midi.
If no insurance is on file, you will receive a bill from Midi for the full cost of your visit and/or treatment.
Q: How will Midi handle self-pay patients who live in Minnesota?
A: While it is true that Herself Health handles billing of Midi visits for MN patients with in-network Blue Cross Blue Shield PPO insurance, self-pay MN patients will continue to get billed by Midi.

Q: When Herself Health bills me for my Midi visit, what information will the bill contain?
A: If you live in Minnesota and have a primary in-network Blue Cross Blue Shield PPO plan on file with Midi, Herself Health will handle billing insurance for your Midi visit on our behalf.
You will receive a bill from Herself Health, rather than Midi, for any cost-sharing you are responsible for (e.g., copay, deductible, co-insurance, etc.)
The bill will note:
- Herself Health’s partnership with Midi
- Reference ID number
- Visit fees and amount covered by insurance*
- Treatment fees and amount covered by insurance*
- The amount you owe*
- How to pay Herself Health online or by check
- Customer service phone number

Q: Do I have to pay extra if my Midi visit is being billed via Herself Health?
A: No. Patients do not pay anything extra because of Midi’s partnership with Herself Health.
When billing MN residents’ primary in-network Blue Cross Blue Shield PPO insurance plans for Midi visits, Herself Health will process those claims like any other kind of specialty care.
You will receive a bill from Herself Health for any cost-sharing under the terms of your plan. Payments should be sent to Herself Health, not Midi.
Q: Herself Health billed me for my Midi visit. Who do I pay?
A: Payment for any cost-sharing related to a Midi visit billed by Herself Health should be sent to Herself Health. Midi cannot process these payments.
Q: How can I pay my Herself Health bill for my Midi visit?
A: You have two options for paying Herself Health bills for Midi visits:
Q: What is Herself Health’s turnaround time for filing claims with Blue Cross Blue Shield-MN?
A: Herself Health will file claims with primary in-network Blue Cross Blue Shield-MN in accordance with their standard claim submission timeline, which is usually 3 business days following the visit.
Q: Are Midi supplements and Midi CustomRx billed through Herself Health, too?
A: Midi supplements and Midi CustomRx are cash-only services and not covered by any insurance plan. Patients pay for these products upon ordering.
Q: I’ve decided to visit a Herself Health clinic for follow-up clinical care. Do they accept insurance?
A: You can view a complete list of the insurance plans accepted by Herself Health on their website.*

Q: Who do I contact if there is a delay in Herself Health filing a claim with my insurance for my Midi Visit?
A: Herself Health files all Midi visit claims for MN residents with primary in-network Blue Cross Blue Shield PPO plans on file with Midi.

If needed, Herself Health will escalate the issue to the Billing team at Midi.
Q: Who do I contact if a claim for a Midi visit billed to my insurance by Herself Health is denied?
A: If you are a Minnesota resident with primary, in-network Blue Cross Blue Shield PPO insurance on file with Midi and are concerned about denial of a claim filed by Herself Health for your Midi visit, please email HHmidibilling@herself-health.com. If needed, Herself Health will escalate the issue to the Billing team at Midi.

A: If your insurance information has changed, please upload your updated plan information under “Insurance Information” in the Account section of your Midi account.
